Prepare the records and access for a professional visit
Use this checklist to organise an enquiry and appointment. Completing it does not assess appliance safety or replace the provider’s instructions.

Collect information already available
Find the owner manual, earlier invoices and written findings if you have them. Note what is missing without guessing a model, service date or diagnosis. A photograph from an ordinary accessible position can help identify the appliance and explain the room layout. Leave inaccessible labels and internal equipment for the professional.
Write down the reason for the request in plain language. Keep an existing instruction about use of the appliance with the relevant report. Do not relight, dismantle or test equipment to complete this checklist. For an immediate safety concern, use the current official emergency guidance instead of waiting for an enquiry response.
Agree the practical arrangements
Confirm who will provide entry, who can approve the scope and how the provider will contact that person during the visit. Tell the provider about occupied rooms, pets, keys and any managed-building access arrangements. Ask what preparation is needed before moving heavy items or making changes around the appliance.
After the visit, keep the completion record and any further recommendation together. Ask which work was completed, which matter remains unresolved and what follow-up has actually been agreed. A checked box in this planner records your preparation, not professional certification of the heater or installation.
Your preparation record
Tick only the preparation tasks completed. This does not certify the appliance.
